For an appointment at the WHS Spay & Neuter Clinic, you must complete ALL fields. If you have more than 2 pets you will need to fill out the form for each pet. We will make every effort to schedule dates together for multiple-animal households. Your appointment slot will be confirmed by a follow up email within 24 hours on our working days (Monday - Friday) so make sure to check your SPAM filter if you have not heard from us within that timeframe.
