For an appointment at the WHS Spay & Neuter Clinic, you must complete ALL fields. If you have more than 2 pets you will need to fill out the form for each pet. We will make every effort to schedule dates together for multiple-animal households. Your appointment slot will be confirmed by a follow up email within 24 hours on our working days (Monday - Friday) so make sure to check your SPAM filter if you have not heard from us within that timeframe. PLEASE DO NOT ALSO CALL FOR AN APPOINTMENT IF YOU FILL THIS FORM OUT.
